HemorrhageFrankelEtAlPhysiologic20

Description

In Frankel, et al. 30 mL/kg (43% blood loss) of venous blood was removed from the pig to produce shock (defined by MAP of 20 mmHg). Shock was defined as maintaining MAP at less than 20mmHg for an hour. Resustitation was then started using Ringer's Lactate and whole blood with a ratio of 2:1 (28ml/kg Ringer's, then 14 ml/kg whole blood) to reach and mantain a MAP of 60mmHg for 120 minutes. The goal of this scenario was a physiologic hemorrhage rate over 20 minutes (2.15 mL/kg/min for 7 min and 1.15 ml/kg/min for 13 min).

To replicate this, 43% blood loss in the standard female is approximately 1800 mL or 30.5 mL/kg. This is close enough to 30ml/kg.

Segment Validation

Segment 1

Lungs were mechanically ventilated in volume-controlled mode with a FiO2 of 0.21 and a PEEP of 5 cm H2O. Tidal volumes were adjusted to 7 to 10 mL/kg at 15 breaths per minute (fixed rate) to achieve a PaCO2 between 35 mmHg and 40 mmHg.
